Participation in the Precision Care Program only requires that you are a member of the FMCP, are currently prescribed five or more prescriptions, and authorize us to have your prescriptions transferred to ensure the continuous oversight and support of the Program.

How will I know of any changes to my prescriptions as a result of the PGX test results?2021-01-26T02:15:43+00:00

The personal pharmacist assigned to you will review your Pgx test report with you, including any related changes made to your medications by your prescribing doctor(s).

This level of continuous support will apply to any future prescription changes.

You will not need to tell them anything.

We will reach out on behalf of FMCP to introduce ourselves and describe the Program’s value for both you and your prescribing doctor(s).

Do I need to ask the pharmacy I use now to ensure my prescriptions are transferred?2021-01-26T02:30:42+00:00

No. Once you are registered in the Program, we will arrange to have all of your current and future prescriptions transferred to our pharmacy partner.

Absolutely, your medical record will always be under the protection of the HIPAA law.

You are the only one who can authorize medical personnel to access your personal health information in order to provide their services to you.

Only your prescribing physician(s) and our pharmacy partner.

How often do I need to repeat the PGX test?2021-01-26T02:33:12+00:00

Because your genetic data does not not change over time, you will not need to repeat the test.

No, there will be no additional cost to participants. The FMCP is subsidizing any additional program costs to ensure all qualified members have equal access to it.
